FELLOWSHIP Application

Instructions for completing the application are as follows:

  1. Read the entire set of instructions carefully before filling out the application.
  2. Type all the information; the application can be filled out online or as an editable Adobe Form which can be completed with any Adobe Reader.
  3. Sign and date the waiver statement provided on the Reference Form and send one form to each individual listed in Section V.

Application Checklist

Include the following supporting credentials:

  1. Personal Statement of not less than 300 nor more than 500 words indicating why you are interested in a career in the field you have chosen.
  2. If ever in the military service upload at the end of the online application, email, or mail a copy of the DD214.
  3. Arrange for official transcripts of credits to be forwarded by the registrar of each college/university you have attended.  Transcripts must be received no later than November 15th.  Official Transcripts must be  received directly from the college/university. 
  4. Send official transcripts no later than November 15th to:
