Instructions for completing the application are as follows:

1. Read the entire set of instructions carefully before filling out the application.
2. Type all the information; the application can be filled out online or as an editable Adobe Form which can be completed with any Adobe Reader.
3. Sign and date the waiver statement provided on the Reference Form and send one form to each individual listed in Section V.

Second and Third Year Application

1. If ever in the military service upload at the end of the online application, email, or mail a copy of the DD214.
2. Arrange for official transcripts of credits to be forwarded by the registrar of each college/university you have attended.  Transcripts must be received no later than November 15th.  Official Transcripts must be  received directly from the college/university. 
3. Send official transcripts and DD214 no later than November 15th to:
